Transoral Thyroidectomy

Technique and Evolving Indications

The technique for transoral thyroidectomy vestibular approach is described in more detail elsewhere in this text and is briefly outlined here. The most adopted endoscopic technique as described by Anuwong [1] describes three incisions inside the lower lip within the oral vestibule: a 1- to 2-cm middle transverse incision just above the frenulum and two 5-mm longitudinal incisions in the lower lip just anterior to the oral commissures and within the mucosa at 1 mm of the vermilion border. For each incision, a tract is created through the muscle layer (mentalis for the midline port, orbicularis oris and/or depressor anguli oris for the lateral ports) into the subcutaneous tissues up and over the mandible in order to enter into the subplatysmal space in the upper neck, which is opened widely first with or without hydrodissection and blunt dilators followed by skin flap elevation under direct vision after the placement of endoscopic/laparoscopic ports (Fig. 10.2).
